📌 Required Documents for New Passport

According to the U.S. Department of State:

  • Form DS-11 — Completed but not signed until instructed by an acceptance agent
  • Proof of U.S. citizenship — Original or certified copy of birth certificate, naturalization certificate, or previous U.S. passport
  • Photo ID — Valid driver's license, government ID, or military ID
  • Passport photo — 2x2 inches, white background, taken within last 6 months
  • Fee payment — Separate checks or money orders for application and execution fees
📌 Passport Renewal Eligibility

You can renew your passport by mail using Form DS-82 if:

  • Your most recent passport is undamaged and can be submitted with your application
  • It was issued when you were 16 years of age or older
  • It was issued within the last 15 years
  • It was issued in your current name (or you can document your name change)

According to travel.state.gov:

"If you have a life-or-death emergency and need to travel within 72 hours (3 business days), contact the National Passport Information Center at 1-877-487-2778 to make an appointment at a passport agency or center. You must provide proof of the emergency and international travel."
